2026-2027 Season Required Documents/Forms

*LTHS does NOT share documents with Lyons Township Hockey Club

• USA Hockey Safe Sport Certification for players who are born in 2009 or earlier (required before a player can be rostered on a team. Must be completed by August 30, 2026).
• Code of Conduct (signed as part of the online registration process)
• Required to be emailed to the club before the Parent/Player meeting -
• LTHS 2026-2027 Student ID (provide copy)
